CONTEST RULES:
Survey and contest entry closes at midnight on January 31, 2015. Only one survey/entry per person. Multiple submissions will void your contest entry. Winners will be chosen at random and announced and contacted via email the following week. While anyone can take this survey, the contest is only open to TN residents age 18 and older, and prize packages must be picked up at our home office in Murfreesboro, TN. No purchase required.
